Dosing then range from 60 mg/kg/day to 80 mg/kg/day divided q6h and monitoring with serum trough level usual maximum 3600 mg/day according to patient weight.

Vancomycin maximum dose is different according to the indication, some indications have maximum dose of 2000mg/day, others have 3600mg/day as a maximum.
