Indications for RBC transfusion for hospitalized patients:

  • Transfuse when Hb level is < 7 g/dL (70 g/L) for all patients (Strong recommendation). This recommendation applies to all except patients who:
  1. Are hemodynamically unstable
  2. Have stable cardiovascular disease or acute coronary syndrome
  3. Are having orthopedic or cardiac surgery
  • Consider transfusion when Hb level is ≤ 8 g/dL (80 g/L) for patients with stable cardiovascular disease or those having orthopedic surgery.

In pediatrics, Red cells are transfused at up to 5 mL/kg/h (unless there is active major bleeding) and the transfusion should be completed within 4 hours
