First of all it’s not an absolute contraindication. Use with caution. Limited, low-dose therapy is usually well-tolerated in hepatic disease/cirrhosis; however, cases of hepatotoxicity at daily acetaminophen dosages <4,000 mg/day have been reported. Avoid chronic use in hepatic impairment.

IV paracetamol leads to rapid and higher plasma levels than oral forms.

In severe liver disease, the liver’s capacity to metabolize the drug is impaired — increasing the risk of hepatotoxicity even at therapeutic doses.
