Oral: 1 to 2 mg/kg/day once daily; maximum daily dose: 40 mg/day.

IV: 0.8 or 1.6 mg/kg once daily; maximum single dose: 80 mg; dosing from a single dose pharmacokinetic study in 18 patients (age range: 2 to 14 years) (Kearns 2008).
