Give 40 mg/kg/dose at the recommended age-specific dosing interval
Less than 32 weeks GA and less than 14 days PNA: every 12 hours .
Less than 32 weeks GA and 14 days PNA and older: every 8 hours .
32 weeks GA and older: every 8 hours .
