Calculate the dose as usual with the usual formula:

Vol = dose (mg) x wt (kg) / conc (mg/ml)

The trick here is to make sure that the meropenem will be stable during the 4 hours of administration. This can be achieved by reconstitute the meropenem in NS to a concentration of 10 to 14 mg/ml
