preclinical studies support neuroprotective roles for allopurinol and erythropoietin, but pediatric clinical evidence is lacking.
Allopurinol has shown some promise in infants with asphyxial injury or surgical ischemia, and EPO was found to be neuroprotective in animal models, yet large trials have not confirmed benefit in human neonates or adults.
Both agents remain investigational for post-arrest care. Until further evidence emerges, treatment guidelines recommend focusing on established post-resuscitation measures (oxygenation, perfusion, temperature control) rather than unproven pharmacotherapies.
