Has she been ventilated?

Cap or vap pneumonia?

Chest xray does it suggest viral infection?

Sputum colour , comorbidity copd asthma ?

According to each scenario it may leads you to causative organism

For cap it is sufficient to use only levofloxacin as it will covers typical and atypical bacteria which they are the causative organism , adding ceftriaxone may not lead for additional documented benefit for cap pneumonia

Combination of macrolides and levofloxacin will only lead to qt prolongation with no broader coverage or synergism
