Garamycin (gentamicin) inhalation is not a widely approved or commercially available formulation, but it is sometimes used off-label in certain clinical situations via nebulization.

Indications of Garamycin (Gentamicin) Inhalation:

  1. Chronic Pulmonary Infections in Cystic Fibrosis (CF):
    • Used as an off-label alternative to inhaled tobramycin.
    • Targets Pseudomonas aeruginosa colonization or infection.
    • May be considered in settings where tobramycin inhalation solution is not available or not tolerated.
  2. Ventilator-Associated Pneumonia (VAP):
    • As adjunctive therapy for multidrug-resistant (MDR) gram-negative organisms.
    • Delivered through nebulization in ICU settings.
    • Usually in combination with systemic antibiotics.
  3. Non-CF Bronchiectasis with Recurrent Infections:
    • Off-label use in patients with chronic Pseudomonas colonization.
    • Intended to reduce bacterial load and exacerbations.
  4. Mycobacterial Lung Infections (e.g., Mycobacterium abscessus):
    • Occasionally used off-label as part of a multi-drug regimen.


Important Notes:

  • Formulation: Gentamicin IV (Preservative Free Formulation) solution is typically used via nebulizer when no dedicated inhalation product is available.
  • Preparation: Must be done under aseptic conditions. Doses are adjusted for inhalation use.
  • Toxicity Risk: Gentamicin can be nephrotoxic and ototoxic, though systemic absorption from inhalation is generally low. Serum monitoring is rarely needed unless used with systemic aminoglycosides. Toxicity to the lung tissue may be a concern especially when used for long periods.
  • Side effect: generally any antibiotic inhalation come with the risk of bronchospasm that may require the use of inhaled bronchodilators (such as salbutamol) just before the antibiotic inhalation session.
