In pediatric patients, DVT (deep vein thrombosis) prophylaxis is not as routine as in adults and is usually considered only in high-risk cases due to the overall lower incidence of VTE (venous thromboembolism) in children and higher bleeding risk.

you should assess:

  1. Does this baby have a central venous catheter?
  2. Is the baby severely septic or with multiorgan dysfunction?
  3. Any history of thrombosis or family history?

If yes to multiple factors, low-dose LMWH may be cautiously considered with hematology consultation.

Risk Factors That May Justify DVT Prophylaxis in Pediatrics:

  • Central venous catheter (CVC) Especially femoral or jugular lines—biggest risk factor
  • Mechanical ventilation Prolonged ventilation is a moderate risk, especially in critically ill
  • Genetic or acquired thrombophilia e.g. Protein C/S deficiency
  • Severe infection or sepsis Elevates inflammatory state and clotting
  • Congenital heart disease Especially post-surgical patients
  • Prolonged immobility May contribute in older infants or those sedated for long
  • Personal/family history of VTE Increases risk
  • Dehydration or polycythemia Increased viscosity of blood
  • Obesity (in older children) Not relevant here
  • Malignancy Rare in this age but high risk if present
