When to return to normal dose:

  • Not just when S. Creat drops once.
  • Return to normal dosing only if:
  • S. Creat is stable or decreasing over 48 hours.
  • Urine output is normal (generally > 1 mL/kg/hr).
  • No ongoing renal hypoperfusion (e.g., no new hypotension, hypoxia, or nephrotoxic exposure).
  • Drug level monitoring (if applicable) supports normalization.

Suggested practice in your case:

  • Since S. Creat dropped to 0.9 then rose again to 1.3, that suggests transient improvement — not sustained recovery.
  • Keep renal-adjusted dose until S. Creat is:
  • <1.0 and stable for 48 hrs, and
  • Clinical signs support renal recovery.
