Neonate 38 w with pulmonary stenosis & CHD
S. Creat 0.7 then 1.2 then 1.3 then 1.7 I made renal dose adj  for AB then decreased to 0.9 I return to normal dose then it increased to 1.3
Q : When I should return to normal dose ?!

When to return to normal dose:
- Not just when S. Creat drops once.
- Return to normal dosing only if:
- S. Creat is stable or decreasing over 48 hours.
- Urine output is normal (generally > 1 mL/kg/hr).
- No ongoing renal hypoperfusion (e.g., no new hypotension, hypoxia, or nephrotoxic exposure).
- Drug level monitoring (if applicable) supports normalization.

Suggested practice in your case:
- Since S. Creat dropped to 0.9 then rose again to 1.3, that suggests transient improvement — not sustained recovery.
- Keep renal-adjusted dose until S. Creat is:
- <1.0 and stable for 48 hrs, and
- Clinical signs support renal recovery.
