10-y old girl with disturbed conscious level, PH 7.27 , CRP 48
TLC 10
GRAN% 18.7
Controlled temperature, severe secretions
No blood culture available yet
ON ceftriaxone for 1 day, Physician changes for meronam and vancomycin.
Are these conditions indication for Meronam and Vanco?

She is presented with pneumonia for the second time, history of one time admission (NICU admission), slight difficulty in learning with no follow-up with neurology Physician, no suiezers.
Recent bleeding with suction, coagulation profile is requested, but result is not available yet.
As Dr.Hossam opinion normal tlc would indicate it may be viral pneumonia, ceftriaxone is a good choice, you can have BAL culture with follow up
Certainly vanco has no indication unless it is vap and late onset also better DC.
Check dosing and administration also
With supportive care
What is her pre admission history? Did she admitted before?
Is there any definite focus of infection? For example, presented with pneumonia, Gastroenteritis, … etc.
Why giving her antibiotics at all ?
If you present a full comprehensive history, so I can understand your questions better.
Thank you.
