There are no data available for pediatric patients with renal impairment; however, the following dose adjustments are based on recommendations for adults with renal impairment:
50 to 79 mL/min: 2.5 to 3.8 mg/kg/day divided into 2 doses per day
30 to 49 mL/min: 2.5 mg/kg once daily or divided into 2 doses per day
10 to 29 mL/min: 1.5 mg/kg every 36 hours
